Teresita N. PEREZ
Teresita peacefully passed at Humber River Hospital; she was a resident of West Park Healthcare Centre since 2010. Predeceased by her parents Anacleto and Felicidad (née Constantino) Natividad, her husband Fortunato, her sister Yolanda, and her brothers Virgilio (Azucena) and Francisco (Conchita). Survived by sisters Gloria (Martin Ignacio), Rosalina (Rolly Asuncion (deceased)), Remedios (Eddie Ancheta), Virginia (Polly Marcos), Ma. Luisa (Lito Martinez) and Concepcion (Ray Notario (deceased)), brother Rufino (Marina) and nieces and nephews. Mother of Joselito (Linh), José Mari and Jose Gilbert. The family thanks the staff at West Park Healthcare Centre for their incredible caring support over the years, as well as the nursing staff at Humber River Hospital during her final days. Visitation will be held at the Jerrett Funeral Home – St. Clair Chapel, 1141 St. Clair Avenue West, Toronto (1 block east of Dufferin Street), on March 24, 2017, from 6 to 8 p.m. The funeral service will be held in the Chapel on March 25, 2017 at 1 p.m. Cremation to follow the service, with interment at Prospect Cemetery and Mausoleum, Toronto.
